Complete Your PRIVIT Information

PRIVIT Checklist - Fall 2022

1. Create your PRIVIT account.
2. Join the Team titled, "2022 Fall New/Transfer/Re-Admitted" (password: NEW)
3. ATHLETES - Join the appropriate sports team.
4. Complete the general information form.
5. Complete the emergency medical Form.
6. Complete the student medical form.
7. Upload your immunization records or approved accommodation form.
8. Upload your COVID vaccination records or approved accommodation form.
9. Upload your insurance card (front and back).
10. ATHLETES - Complete Sport Required Forms
11. Email accommodation forms to wchealthcenter@wilmington.edu for approval. Once the accommodation form is approved, the form will be emailed back to you. Upload your approved accommodation form.
